Establish why the varicocele matters
A varicocele involves enlarged veins around a testicle, but its presence does not automatically mean treatment will improve fertility. Tell the urologist whether the consultation is about discomfort, a fertility concern or an incidental finding. EAU guidance distinguishes a clinically assessed varicocele from one found only through imaging and connects fertility treatment recommendations to the wider evaluation. Ask how the finding was established in your case and what evidence links it to the problem you want addressed. The decision should follow that reasoning rather than the availability of a procedure in a travel package.
Include both partners’ circumstances
If fertility is the goal, ask the specialist to interpret semen results alongside the relevant assessment of your partner. Discuss whether other explanations for difficulty conceiving have been considered and how treatment timing fits the overall fertility pathway. Request an account of the expected benefit and its uncertainty, avoiding a promise of pregnancy from correcting a single finding. Ask about the proposed technique, possible complications, recurrence and alternatives. If the recommendation is observation or another fertility approach, have the clinician explain what would prompt reconsideration rather than assuming that no operation means nothing is being done.
Plan the assessment of outcome
Before travelling, establish which follow-up visits and tests will be needed to judge the result. Ask where they can occur, who will interpret them and how later decisions will be coordinated with a fertility clinic. Keep the examination findings, imaging and semen reports together so a second specialist can review the actual case. Clarify the costs of continuing assessment and any further intervention if symptoms persist. Comparing a complete pathway helps you see whether an overseas procedure adds a useful option or creates a gap between the operation and the fertility care it was intended to support.
Questions to take with you
- Is this a clinical varicocele relevant to my symptoms or fertility findings?
- How does the recommendation fit both partners’ assessment?
- Who will review the postoperative semen results and next steps?
