Include removal in the original discussion

An internal lengthening nail may require a later removal operation. Ask about this when comparing treatment pathways, rather than assuming the initial surgery covers every device-related procedure. The FDA’s 2023 Precice communication includes removal instructions for the specific systems it addresses. Your surgeon should explain the current instructions for the exact implant proposed and how they fit your care. Do not use a general internet timeline as a personal removal date. The relevant discussion connects manufacturer guidance, regulatory information, clinical assessment and the condition of your bone.

Establish who will take responsibility

Ask whether the original surgeon expects to remove the nail and what records another surgeon would need if care transfers. Confirm access to the appropriate equipment and familiarity with the implant before assuming that a local service will accept the procedure. Request the device identifier, operation report and relevant imaging after the original surgery. If removal is included in a package, clarify what that covers: assessment, hospital fees, anaesthesia, the operation and follow-up may be treated separately. Also ask how the arrangements change if removal proves more complex than anticipated.

Plan for another period of care

Treat the later operation as its own medical-travel decision. Ask about the expected assessment, individual activity restrictions and support needs, without presuming it will resemble the first recovery. Check who advises on work, travel and rehabilitation afterwards. If your circumstances change between operations, discuss that early rather than waiting until the proposed date. Keep a reminder to arrange the clinical review, not simply a calendar booking for removal. The objective is a continuous pathway in which the implant’s future is accounted for, including who will contact you if relevant device guidance changes while it remains in place.

Questions to take with you

  1. What do the current instructions say about removal of my exact nail?
  2. Who can perform removal if I cannot return to the original centre?
  3. Which assessment and aftercare costs are included?
