External-frame pin care: establish one consistent routine

Arrange teaching and supplies for the actual frame before leaving hospital.

Learn the method used by your team

External frames have pins or wires that pass through the skin, creating sites that need care. NHS specialist services provide teaching tailored to their own protocols. Ask your team to demonstrate the routine for your frame and supervise you or your companion doing it. Request written instructions in a language you understand. Online videos from another hospital may use different products or methods, so ask before combining them. The practical aim is a routine you can follow consistently, with enough understanding to recognise when something has changed and needs clinical review.

Make the supply chain work abroad

Obtain the names and quantities of the items your team expects you to use, along with a plan for replacement. Check whether supplies are included, where they can be obtained locally and how they will be stored in your accommodation. Confirm arrangements for washing, keeping the area organised and disposing of used materials. If your stay is extended, identify who renews the supply order. These questions are especially useful when products have unfamiliar local names. Keep a photograph of the labelled packaging for communication, while asking the team before substituting an apparently similar item.

Agree how concerns will be assessed

Ask which changes at a pin site require contact and how urgently, including new pain, redness or discharge. Establish where an examination can take place rather than relying entirely on uploaded photographs. Do not start leftover antibiotics or change cleaning products without advice. Before moving accommodation or returning home, confirm that the next care team accepts the frame-care plan and can obtain the recommended supplies. Record any treatment for a pin-site problem so that later clinicians can see what was assessed and prescribed. A clear record helps when several people share care over a prolonged period.

Questions to take with you

  1. Can you supervise me practising your pin-care method?
  2. How will I obtain replacement supplies during an extended stay?
  3. Where will a concerning pin site be examined promptly?
